    IMPORTANT: To successfully renew the 4S Label, you are recommended to submit the documents showing the continuous adoption and implementation of the 4S devices according to the "Annex A - Recommended 4S packages" of the document "Application Guide for 4S Labelling Scheme" or any other 4S devices being applied to address respective risks identified by the safety officer based on a comprehensive risk assessment.

    In order to shorten the time in filling this application form, you are recommended to prepare the following documents in electronic format for submission before start:

     

    1. 4S Product Information (for non CITF pre-approved SSSS Product only)
       (e.g. product catalogue or specifications or user guide or installation manual etc.)

    2. Project scope of works

    3. Location plan(s) for the 4S devices
      If the 4S device has installed in a fixed location (e.g. E-lock, A.I. Camera, proximity sensors etc.), engineering level floor plan(s) to demonstrate the proper implementation of 4S solutions are required.

    4. Operation arrangements or other related documents
      If the 4S device has not been installed in a fixed location (e.g. smart wearables, asset tags etc.), a document describing the use of the solutions is required.

    5. Other supporting document(s) (Optional)
       (e.g. risk assessment, 4S implementation plan, safety plan etc.)

  • I,   *        being an applicant for 4S Labelling Scheme hereby DECLARE that the above is a true statement of my particulars, that I have read and understood the Guidelines as stipulated by the Construction Industry Council (CIC), and I do hereby accept the final decision of the Subcommittee of 4S Labelling Scheme of CIC.


    I undertake that, in the event of any change in the above particulars, I will make known the changes, within 14 days, in writing to the Subcommittee of 4S Labelling Scheme.


    I understand and authorise CIC to make any enquiries and check all information in relation to my application for 4S Labelling.


    I acknowledge that CIC has the right to withdraw approval of application for 4S Label if I do not meet the requirements. I understand and agree that CIC may investigate the statements I have made with respect to this application, and that I may be subject to disciplinary actions for any misrepresentation (whether fraudulent or otherwise) in this application.


    If at any time CIC discovers that I have failed to disclose any pertinent information in this form, or that I have provided false information, it will have the right to terminate my application with immediate effect.

  • Personal Information Collection Statement

     

    1. Collection of Personal Data

    1.1.   The information you provide to the Construction Industry Council, its affiliates, and / or its subsidiaries, including, but not limited to, the Hong Kong Institute of Construction and Zero Carbon Building, Construction Sector Imported Labour Quarters Limited (collectively, the “CIC”), including any personal data as defined in the Personal Data (Privacy) Ordinance (Cap.486), will be used solely for purposes related to the activities of the CIC. The activity and the required personal data are detailed in the application form.

     

    1.2.   Whether or not you provide your personal data to the CIC is voluntary. However, where you are providing information for the purpose of an application, it is necessary that you supply the CIC with complete information as specified on the application form. Otherwise, the CIC may be unable to process or consider your application. If you are under the age of 18, you should consult your parent or guardian before providing any personal data to us.

     

    1.3.   You are entitled to request access to and correction of any errors in your personal data. If you wish to do so, please write to the Construction Digitalisation Department (Data Access Request), Construction Industry Council, 38/F COS Centre, 56 Tsun Yip Street, Kwun Tong, Kowloon.     2. Purposes of Collection

    Your personal data will be kept confidential and may be used by the CIC and the Development Bureau (DEVB) for the following purposes:

     

    A) Collection Purposes

    1. Assessing your application to 4S labelling Scheme; and
    2. Serving as reference materials of the construction industry for the purpose of promoting technology adoption and the 4SLS. Approved project information (including but not limited to the contract title, site location, contract name, developer or project client’s name, the approval date of the 4S Label, photos, documents, reports, or videos submitted with the application) will be published on CIC public domain including CIC website, seminar, or other promotional materials for public information.
    3. Facilitating communication with you;
    4. Performing and exercising functions and powers of the CIC under relevant legislation, rules and sub-legislation, including but not limited to the Construction Industry Council Ordinance (Cap.  587) and Construction Workers Registration Ordinance (Cap. 583);
    5. Establishing, exercising and defending the CIC’s legal rights, and complying with the CIC’s legal and regulatory obligations (including anti-money laundering obligations, complying with orders by courts or regulators, etc.);
    6. Managing access to the CIC’s premises and for security purposes;
    7. Preventing and responding to actual or potential security threats, fraud or illegal activities;
    8. Handling complaints or enquiries;
    9. Performing analysis and conducting research and surveys;
    10. Performing audits and compliance reviews to ensure compliance with the applicable CIC’s policies and procedures, regulations and law;
    11. Other purposes related or incidental to the conduct of the CIC’s activities; and
    12. Any other purposes that you may consent to from time to time.

     

    B) Points to Note

    Where your 4S labelling application has been approved, the Approved Project information (including but not limited to the title of the Approved Project and type(s) of technology adopted) will be published on 4S Website for public information.

    It is obligatory for you to provide the CIC with your personal data for publishing. Otherwise, we will not be able to process or consider your application.

     

     

    3. Disclosure and Transfer of Personal Data

    3.1 The CIC and DEVB may disclose or transfer your personal data for the purposes as stated in paragraph 2 to third parties, including but not limited to the following:

    1. Any or all of the CIC’s and DEVB's affiliates and/or subsidiaries;
    2. Any third party service providers, contractors/sub-contractors that, on behalf of the CIC, operate or maintain membership, event registration, tour booking, researches and/or analysis, or carry out back-end services, administrative services, verification services, cloud services or information technology services, or provide necessary support or services to the CIC to enable us to provide our services, including any insurance, banking or third party payment gateways services used by the CIC, and any other entities that discharge contractual obligations on our behalf;
    3. Any of the CIC’s and DEVB's professional advisors, including but not limited to lawyers, accountants and auditors; or
    4. Any party that owes a duty of confidentiality to the CIC and DEVB.

     

    3.2 We may disclose and transfer your personal data in accordance with any legal or regulatory requirements or any court order applicable to the CIC and DEVB.

     

     

    4. Use of Personal Data Related to Direct Marketing

    To keep you informed of CIC activities and developments in the construction industry which may be of interest, the CIC would like to use your personal data, including your name, phone number, correspondence and email address, to update you in relation to CIC training courses, trade testing, registration, events and other aspects of its work and developments in the construction industry.

     

    You are free to decide whether you wish to receive such information. If you choose not to receive information on the above, please put a tick in the box below. You may make any subsequent changes on your choice of receiving promotional materials by writing to us.
